New GOS Referral / Notification Form
A new GOS referral / notification form has been developed by the Sheffield LOC. It is based on a format that has been used in Bradford and Airedale for the last five years to the satisfaction of Optometrists, GP’s and Secondary Care.
The format follows deliberations by a College of Optometrists working group which included representatives from the College of General Practitioners, the Royal College of Ophthalmologists, the AOP and the Department of health.
It is hoped that this new form will encourage clear and succinct referral / notification letters which will improve the care of our patients.
