Please Note Changes to Cataract Assessment and Referral

AS THE FORM STATES,FROM NOW ON ALL CATARACT referrals WITH CO PATHOLOGY SHOULD BE SENT VIA GOS 18
THE FORM IS FOR STRAIGHTFORWARD CATARACT ONLY REFERRALS
GOS 18 referrals with cataract MUST STATE Why they are not being scored – IE: that there is co pathology.
Practices can use the new form straight away by printing off copies from the website, or can still use the older form, BUT must use the new Guidelines. The PCT will be providing a limited number of new forms when they are printed.
Scoring is as before;
In cases of genuine exceptional circumstances you may consider an Individual Funding Request (IFR). It should be noted that the application may not be successful and patient expectations should be managed accordingly.
What you need to do now:
1 Remove all previous versions of the assessment and referral forms. If these are submitted they will be returned to you.
2 Make sure everybody in the practice is aware of the changes.
3 Put a copy of the summary in each testing room and make sure all locums know about it.
4 Keep the copy of the IFR process for reference.
5 When you have an IFR complete the form and send it to

If the application is successful you will receive a letter from the IFR department. This must be attached to your assessment/referral form when you send it to the Cataract Unit.
Please note
The instructions at the bottom of the assessment/referral form now ask you to send the pink and blue copy to NHS Sheffield but give no address.
